New Patient Registration

We welcome new patients who live within our catchment area, please click on the 'Practice Area' tab above to check if you are residing within our catchment area. 

Please Note due to current high volumes of new registrations, the process may take a little longer than a few days at this time.

Your NHS Medical Card

If you have a medical card with your NHS number on it, it would be extremely helpful to the registration process if you bring this to the surgery. The information on the medical card will speed up the process of getting your records from your previous practice. If you do not have a medical card, you will need to obtain your NHS number from your last GP before you are able to register.

Photo ID

Either a passport or driving licence and proof of address, however failure to show these documents does not mean that you will not be registered.

Previous Doctors Details

If you have been registered with the NHS before, please bring with you your previous Doctors Name and Address, so that your medical records can be traced easily.

A completed GMS1 form 

This can be downloaded from this site and completed before you attend or the form can be completed when you attend the practice.

What next?

Once you have been registered as a patient your medical records will be transferred to us, and a new medical card will be sent to your home address. You do not need to inform your existing practice that you are moving, although it may be courteous to do so.
