If you wish to register with the PHGH Doctors, you will be asked a few initial questions to establish whether you reside within our catchment area.
Your NHS Medical Card
If you have a medical card with your NHS number on it, it would be extremely helpful to the registration process if you bring this to the surgery. The information on the medical card will speed up the process of getting your records from your previous practice. If you do not have a medical card, you will need to obtain your NHS number from your last GP before you are able to register.
Either a passport or driving licence and proof of address, however failure to show these documents does not mean that you will not be registered.
Previous Doctors Details
If you have been registered with the NHS before, please bring with you your previous Doctors Name and Address, so that your medical records can be traced easily.
A completed GMS1 form
This can be downloaded from this site and completed before you attend or the form can be completed when you attend the practice.
Once you have been registered as a patient your medical records will be transferred to us, and a new medical card will be sent to your home address. You do not need to inform your existing practice that you are moving, although it may be courteous to do so.
When you register, we may ask you to make an appointment to see the Practice Nurse for a new patient health check. This will ensure that our records are accurate and all routine checks to promote your health and wellbeing (such as blood pressure/smear tests/mammograms) are up-to-date.

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. You will be contacted by phone, and the receptionist will inform you whether or not you are living within the catchment area. If you are living within the catchment area, you will be invited to the surgery to sign your registration form to confirm that the details are correct and to also fill in a Health Questionnaire Form.
Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
